Case Study 4 - Spring Branch Health Center (FQHC) Columbus, OH
A prospective study to determine the efficacy of FQHC based Medical Therapy Management (MTM), through clinical pharmacists working under a collaborative structure with existing primary care providers in a drug management setting. Goal was to improve RX adherence and reduce blood sugar levels for individual and aggregate populations on an assigned basis. Over a twelve-month period, study results indicated a 15% reduction in A1c levels (10.06 reduced to 8.53) and a corresponding increase in RX adherence based on standard industry measures.
